Measurement of the patient’s state of consciousness using validated scales.

Objective:

To determine the patient’s level of consciousness.

Equipment:

– Glasgow scale.

Material:

– Nursing records. – Procedure: – Preserve the patient’s privacy. – Inform the patient. – Request the collaboration of the patient and family. – Assess the patient’s level of consciousness with the Glasgow scale. – Record in the nursing documentation: patient’s state of consciousness, date and time, incidents and patient’s response.

Observations:

If a patient has an abrupt decrease in the Glasgow scale score notify the physician immediately
