Preparation and delivery of prescribed medications vaginally and evaluation of patient response.
Objective:
– Administer prescribed medication to the patient vaginally for therapeutic purposes. – Educate patient and family on therapeutic regimen.
Equipment:
– Tray. – Wedge. – Towel. – Genital hygiene kit.
Material:
– Prescribed medication. – Medical treatment sheet with prescribed medication. – Disposable gloves. – Compresses. – Sponges. – Neutral soap. – Lubricant. – Waste bag. – Sterile gauze. – Genital hygiene material. – Nursing records.
Procedure:
– Perform hand washing. – Follow general rules in the administration of medication. – Determine the patient’s knowledge of the medication and understanding of the method of administration. – Put on gloves. – Place patient in lithotomy position. – Apply the wedge. – Examine condition of perineum, external genitalia and vaginal canal. – Perform genital hygiene. – Separate the labia majora with gauze and locate the vaginal orifice. – Egg administration: 1. Remove the egg from the package. 2. Insert the lubricated ovum through the rounded part into the vaginal canal by inserting the finger in its entirety (8-10 cm).
Administration of creams or ointments:
1.Fill the applicator with the cream or ointment. 2. Insert the applicator 5-7 cm into the vaginal canal. 3. Push plunger of the applicator to introduce the medication. 4. Remove the applicator. 5. Wipe any debris from the labia majora with gauze. – Remove the wedge and put a compress on the patient.
– Leave the patient in a comfortable position. – Collect the material. – Remove gloves. – Wash hands. – Record: medication administered, dose, route, date and time, incidents and patient response.
Observations:
Instruct the patient to remain lying down for 5 minutes after administering the medication.
